Homecare & Rehabilitation Case Manager
We are actively seeking dedicated and compassionate Homecare & Rehabilitation Case Managers to join our team in the UAE’s dynamic healthcare sector.
As a Homecare & Rehabilitation Case Manager, you will provide comprehensive care coordination services to individuals of all ages in their homes.
In this role, you will assess and develop individualized care plans, coordinate with healthcare providers, and ensure the implementation of rehabilitation programs. Working within a multidisciplinary team, you will promote overall health and well-being by providing personalized rehabilitation advice, managing chronic conditions, and addressing patients’ physical, mental, and emotional recovery needs.

Requirements:
- Must have experience in Jawda, Tasneef, DOH & Daman Audit.
- Coding & Nursing knowledge is a must.
- Must be able to handle the operations management of Nurses & Therapists.
- Dealing with ICT codes (Preferred).
- Bachelor’s in Nursing is a must.
- Minimum 2 years of experience in handling Nursing & Therapists’ Operations Management.
- Must have Leadership skills.
- Team building skills.
- Case Management skills.
